About Roots Health

“Putting the Care Back in Health Care”

At Roots Health we provide a straightforward model of healthcare that delivers what matters most: excellent care, personalized service, and access whenever you need it. We believe the patient-physician relationship is fundamental to patient well-being and we have removed third party intrusions our health care model. The vital relationship between patient and physician promotes education, cooperation, and ultimately empowers patients to reach their own personal health and wellness goals with the support they deserve.

Restoring core value to the delivery of health care, we utilize the physician-patient relationship to heal. We provide a solid foundation to reclaim the priorities of health and wellness in our families and communities. Through Direct Primary Care we are inspired to transform the health care experience to deliver the care that everyone deserves.
